We would greatly appreciate your participation in a very important questionnaire to help us capture your feedback, thoughts, and ideas on future program offerings at SEASPAR. The questionnaire contains approximately 20 questions and should take less than 7 minutes to complete.
Your responses to the questionnaire are critical in identifying future improvement to program offerings at SEASPAR. Your time in completion of the questionnaire is greatly appreciated.
Participation in this questionnaire is open to anyone aged 18 years and older. Your participation in this questionnaire is voluntary, and responses will be kept confidential. Your responses are anonymous, and will not identify you individually.
By agreeing to participate in the questionnaire, you agree that we may use this data to understand and improve future program offerings at SEASPAR. Please note that responses to the questionnaire will not eliminate any current program offerings at SEASPAR.
Again, the questionnaire is completely voluntary, and all information provided will be kept confidential. Please complete the questionnaire before March 8, 2017 by 11:59 p.m. Questionnaire results will be compiled by the University of St. Francis, Joliet, Illinois.
